  • exploration, development or operation of oil wells, natural gas wells, strip mines or quarries
  • food processing plant or other industrial/commercial activity requiring wastewater treatment facilities
  • bulk storage of petroleum or asphalt products (10,000 to 80,000 gallons)

  • production or disposal of hazardous waste
  • storage of hazardous waste or materials (except agricultural pesticides)
  • storage of pesticides for repackaging or resale
  • installation of long distance transmission pipelines for natural gas, liquid petroleum, petroleum products, slurry coal, or other liquids/solids (except water, sewer, storm sewers)
  • application to land surfaces or landfilling of industrial, commercial or residential waste
  • relocation of streambeds or channelization of streams (except flood damage correction or conservation plan)
  • establishment of feed lots with more than 100 animal livestock units (unless under best management practices)
  • bulk storage of petroleum or asphalt products in excess of 80,000 gallons
  • storage of petroleum or asphalt products within 400 feet of permanent tributaries or reservoir (except heating oil ≤550 gallons)
  • storage of pesticides within 400 feet of permanent tributaries or reservoir
  • establishment of feed lots within 400 feet of permanent tributaries or reservoir (unless under best management practices)
  • application of biosolids, livestock waste, poultry litter, or other animal waste within 50 feet of permanent tributaries or reservoirs (except with nutrient management plan)
